Attention House League Registrants-Proof of Vaccine Required

From the Ontario Minor Hockey Association (OMHA):

“Earlier this summer we launched an effort to remind everyone involved in minor hockey across the province that our game would be back – that our children, families, association, and communities would come back to the game we all love – Stronger than ever.”    

On behalf of the BMHA board we are excited as the start of the 2021/ 2022 fast approaches and we can get the players back on the ice and enjoying the game of hockey. We want to thank all our members for cooperating with us as we launch our return to hockey while working within the COVID guidelines. We made it through both our rep try-outs and house league evaluations successfully and now it’s time to play hockey!

On September 9, 2021 the OMHA announced a vaccination policy that all members must follow and the date of compliance is fast approaching. The OMHA policy requires that all eligible members (players birth year 2009 and above, team officials, volunteers and association officials) must be fully vaccinated (including a 14-day period after second dose) by October 31, 2021 or earlier.  

All members of the BMHA must provide documented proof to the association of vaccination either by:
1. A digital or physical Dose Administration Receipt
2. Medical record signed by a licensed health care provider indicating vaccine name and date(s) of administration.   
3. Government verification/language (e.g. Government issued vaccine passport card)  

At this time, we do not want you to share this information with your team’s coach or manager and this process will be handled by the BMHA office. If any volunteer or player have already met these guidelines you can share with the BMHA designate either through our online form or if prefer you can stop into the office and we will do a visible confirmation of your status. All records from our members will be kept in a confidential manner and once these are no longer required will be destroyed in a secured manner.   Here is a link to the online form:

On November 1, 2021 any member not fully vaccinated (including the 14-day period after receiving their second dose) will no longer be eligible to participate in any hockey activities. Members have until November 14, 2021 to provide proof of vaccination before they are removed from the team’s roster.
